No Show and Cancellation Policy

At CPHS Wellness Center, we are committed to providing affordable, accessible care. Missed appointments limit our ability to serve other patients and keep costs low for our community.

 

Cancellations & No-Shows

We ask for at least 24 hour notice if you need to cancel or reschedule your appointment.

  • Late Cancellation: Less than 24 hour notice

  • No-Show: Missing an appointment without notice

 

Prepayment Requirement

After two (2) late cancellations and/or no-shows within a 12-month period, a prepayment will be required to schedule future appointments.

  • For appointments: prepayment will be the full cost of the exam fee

  • For surgical procedures: prepayment will be 50% of the procedure cost

    • Surgical deposits may be higher for longer or more complex procedures.

 

Prepayments will be applied to your balance when you attend your appointment.

If you miss or late cancel a prepaid appointment, the prepayment will be non-refundable and forfeited.  It cannot be applied to any future goods or services.

 

Late Arrivals

Clients arriving more than 10 minutes late may need to reschedule and may be considered a late cancellation.

 

Repeated Missed Prepaid Appointments

If Two (2) prepaid appointments are missed or late canceled, we may no longer be able to off future appointments.
